This fast-paced psychological thriller took me on a dark, tense, and twisted journey. I appreciated the new and unusual storyline, despite having to suspend belief with its presentation.
Lily's husband David, is a plastic surgeon who has been asked by a previous mentor to join his clinic that serves the rich and famous. The clinic is on a remote island and comes with a hefty salary that appeals to the young newlyweds. Immediately upon arriving on the island, Lily begins to feel that something isn't right. Security guards, no cell phones allowed, tracking cameras on poles around the neighborhood, and tall white walls around the "compound," have Lily tense and frightened. Hours after arriving she witnesses a woman jump to her death from a balcony, but not before she says to Lily, "You shouldn't have come here, they will come for you."
As Lily begins to see and hear suspicious and disturbing things, she remembers how her mother, who was diagnosed with schizophrenia, behaved and believed. Lily can't decide if becoming like or mother or discovering that these things are reality is the worse of two evils. Can Lily discover what is happening at the clinic? Can she make it out alive?
My thanks to NetGalley, the author, and publisher for an advanced reader copy.
Lily and David are a young married couple, David is a surgeon who gets an offer he can’t refuse : a position as a plastic surgeon for the rich and discrete on a secluded island. The money is amazing and it would help them get started in life. Lily has a very traumatic past, her schizophrenic mom murdered her dad when she was a kid.
As soon as they arrive on the island things are weird. And by weird I mean the red flags are everywhere. Of course there are no cellphones allowed, of course the landline doesn’t allow calls to the outside… Lily starts to get suspicious after a woman tells her she should have never come here before committing suicide (talk about a welcome). There’s also a winery on the island where migrant workers from Mexico come to work.
Can’t blame Lily for being suspicious cause everyone on that damn island is super weird, she should have followed her gut and left right away. But she’s devoted to her (walking red flag) husband and decides to stay which is the worst decision she could make. David is an asshole from the start and some things also don’t add up (a surgeon who can’t find work for months? Come on!) but I really didn’t not expect what actually happens in the last part of the book.
Had to suspend my belief a lot (but maybe people are actually that stupid in real life) but I had fun nonetheless, I really enjoyed both Lily as a character and the « kill the rich » vibe.
An exciting and tense thriller full of shocking twists and turns, I loved this book and found it to be really creepy and shocking! Another fantastic read from James Caine.
David Parker accepts a job on a luxurious private island as their new resident plastic surgeon at Northwind Clinic, a position highly sought after and one that he has been wishing for a for a long time, he is after prestige, acknowledgement and accolades that will all come with being elevated to this kind of role. David’s wife Lily’s world is also about to change as she takes to island life and hopes that all her husbands wishes and dreams are about to come true.
But not long after they arrive, Lily is having serious doubts, something just feels off about this whole set-up. Lily is trying to subdue her fears, all she wants is for her husband to be happy and receive the recognition they feel he deserves. As Lily meets the others working on the island her fears grow, there are security guards and cameras watching everything that is done, she is told because they have such high calibre residents who all value their privacy.
The book moves at fast pace, you know something is very wrong, but initially you are unsure as to what that is. That said the pages turn quickly and soon you are all caught up in this strange world and as the book progresses, I did start to have an idea of what was happening.
James Caine does know how to take you on a thrilling ride so be prepared for the journey you are about to take off on. Thank you to Netgalley, the author and publisher for an advanced copy of the book, all opinions expressed are my own.
The upmarket Northwind Clinic is situated on an isolated island, which promises total secrecy and privacy for its elite clientele. But the costs of its operations are a lot higher than what is printed on the invoices, as Lily Parker discovers when her husband begins his new job there...
Not only does the security at the clinic location start feeling more like surveillance, a woman's sudden death is all the more chilling for the message that she whispers to Lily, warning her that she should not have come there.
What is actually going on at Northwind Clinic? And will Lily survive long enough to find out?
This is a gripping thriller which is also a wild ride. Suspenseful and twisty, it effortlessly holds the reader's attention and gets 3.5 stars.
This storyline was so refreshing. It was different and so new. I loved the it was set on a very remote location and focused on an important topic. Could not get enough of this book. Highly recommend.
This one blew my mind!
A short yet thrilling read on the darkest trade that’s happening with the highest paid surgeons in a clinic!
The writing is really good and the characters are dark as you can expect them to be.
The plot will give you nightmares.
Close the book if you are not in a mood for fast paced thrillers because I did the same when I first pick up the book. But when I picked up the book later, I just couldn’t stop reading it!
Thank you, Twisted Thriller Books, for the ARC.
Dr. Parker gets a call to be the exclusive plastic surgeon for the rich and elite on a private island, along with his wife Lily. Too bad this isn’t a fairytale, with the cameras everywhere watching every single move, coupled with (spoiler) a DEATH on their first night? That’s bad enough. Then when the previous island nurse gives Lily a cryptic message before her death telling her that they shouldn’t have come to the island? It all just proves that instant success comes at a hefty and deadly price. quick rise to the top comes with a hefty price.
This story (and author) had so many twists and turns I could barely keep up. And every character seems to be hiding from something or running from their past. I would love to see this as a mini-series where we can get more in depth into the other characters backstories.
I can’t recommend this enough. Thank you #NetGalley for this ARC
Lily and her husband, David, travel to a secluded island where David will be working as a plastic surgeon to the rich and famous. While the island gives Lily a bad feeling, not having access to the outside world for “privacy” reasons makes her feel even more worried.
I thought this was a fantastic psychological thriller giving me an uneasy, tense feeling while reading. As each secret and twist unfolds, I was glued to the very end! Anyone who enjoys psychological thrillers and mysteries will love this book.
Thanks so much for the opportunity to read in advance!

Lily and David just moved to a remote island where David is going to be a plastic surgeon and make all their dreams come true. Once they get there and see their beautiful condo and the wonderful island, something doesn’t sit right with Lily. On the first night there a woman commits suicidal right in front of lily and tells her “she shouldn’t have come here”. What is going on? Why is everyone acting so strange and why is it impossible to call outside the island?
